Well in Place is only applicable to Delaware and Pennsylvania.

Well in Place is a non-billable service that helps eligible Highmark members in Delaware and Pennsylvania living with serious illness, advanced chronic conditions, and complex life challenges stay supported, connected, and cared for beyond the clinic or hospital.

Many patients struggle not because they lack medical care, but because navigating healthcare, medications, appointments, symptoms, caregiver stress, transportation, or goals of care becomes overwhelming over time. Well in Place exists to help bridge those gaps, working alongside providers as an extension of their care team to deliver coordinated, relationship-based support that is focused on improving quality of life, reducing unnecessary utilization, and helping patients receive the right care at the right time.

Eligibility

In Pennsylvania, Well in Place is available to members enrolled in participating Highmark Medicare Advantage, Medicare Shared Savings Program (MSSP) [Physician Partners of Western PA (PPWPA) providers only], Highmark Individual Affordable Care Act (ACA), Wholecare Medicaid and D-SNP, and HealthyWay (Highmark employees and their covered dependents with Highmark insurance) plans.

In Delaware, Well in Place is available to Highmark Health Options Medicaid and D-SNP members.

Support is provided to members who may benefit from additional medical and psychosocial assistance, including those with:

  • Serious illness: A progressive condition that limits life expectancy and daily function
  • Chronic illness and increased vulnerability due to frailty, functional decline, or advanced age

Services

Well in Place extends support beyond traditional office visits by providing:

  • Ongoing outreach and care coordination
  • Medication review and adherence support
  • Patient and caregiver education
  • Assistance navigating medical and community resources
  • Coordination across providers and care settings
  • Support identifying barriers that may impact the care plan
  • Goals-of-care and advance care planning conversations
