Medical Injectable Drug Program

Last Updated: Monday, December 29, 2025

Physicians can buy and bill for all drugs listed in the Medical Injectable Drug (MID) Program, as all drugs are considered voluntary across all regions, for all lines of business.

If a physician chooses not to buy and bill and the member has the Free Market Health (FMH) benefit FMH will assign a home infusion therapy (HIT) provider or medical specialty pharmacy that is a part of the FMH-contracted network. Providers cannot directly send prescriptions to a specialty pharmacy of their choice for members with the FMH benefit, as they will receive a medical claim rejection.

Please note, not all MID drugs are part of the Free Market Health program. The MID drug list notates which drugs are included in the FMH program.
