Upcoming Changes for Highmark Condition Management Programs

Beginning Jan. 1, 2027, some of our condition management program options for your patients will be changing as we continuously evaluate our offerings to ensure the best possible experience for our members.

The following programs will be discontinued effective Jan. 1, 2027:

• The Diabetes Management Program powered by Onduo and Verily for types 1 and 2 diabetes. Eligible patients with type 2 diabetes may access support through Noom: Weight Management upon meeting eligibility criteria.
• Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD) Management powered by Vida.
• Well360 Virtual Health – Behavioral Health Services: Members will still be able to access the Well360 Women’s Health, Dermatology, and Urgent Care clinics, based on plan type and eligibility. Your Highmark patients using Well360 Behavioral Health may have the option to enroll in Mental Well-Being powered by Spring Health.

Note: You will no longer see these discontinued programs as referral options within Availity Essentials as of Jan. 1, 2027.

Alternative Care and Support Options

There is no requirement to refer to a condition management program. Condition management programs supplement access to care available in our provider networks. We remain committed to ensuring your patients have the resources they need. In addition to seeing in-network providers, Highmark members can also access Highmark’s care management team for extra support. Information about these programs and eligibility criteria can be found on the Provider Resource Center (PRC) pages listed below.
