Credentialing

Step 1: Initial Applicant Registration

Obtain a CAQH provider ID by registering on the CAQH Provider Data Portal and selecting Highmark to receive your information.

Step 2: Join Highmark Network

To enroll as a participant after obtaining a CAQH ID, complete the initial credentialing application via the CertifyOS Provider Portal.

Step 3: Before Billing

Highmark requires providers to be affiliated with a billing practice. To complete this step, use the appropriate request form:

Step 4: Return Contract

After credentialing and adding the practitioner to a group, contracts will be sent. Return the completed contract to Highmark using this form:

Professional Recredentialing

Highmark requires providers to recredential every three years. We will notify the practitioner before it's time.

Organizational Recredentialing

If you have recently received a letter stating that you must recredential, use the CertifyOS Provider Portal to submit your request.
