Two Imaging Codes to Be Removed from Prior Auth List; No Longer Separately Reimbursable

For: Professional and Facility Providers

Effective Feb. 23, 2026, the following two codes, 76376 and 76377, will be added to Reimbursement Policy (RP)-041 Services Not Separately Reimbursed and removed from the prior authorization list: 

  • 76376 – 3D Rendering W/O Postprocessing
  • 76377 – 3D Rendering W Postprocessing

This reimbursement update means that these codes can no longer be billed as a separate service.

In the November 2025 edition of Provider News, this change to RP-041 was communicated to providers.
