Prior Authorization Changes: One Code Added and Nine Removed

Effective Nov. 1, 2026, imaging code 70471 will be added to the prior authorization list applicable to all lines of business.
In addition, nine surgery codes (five vascular and four cardiac) will be removed from the prior authorization list, effective Sept. 1, 2026.
All affected codes are listed below.
