Highmark Now Accepting 275 Claim Attachments via EDI

Effective Jan. 23, 2026, we're significantly streamlining the claims process by accepting electronic 275 Claim Attachment (CA) transactions through Electronic Data Interchange (EDI).

How to Get Started

To begin utilizing 275 CA transactions via EDI, contact your practice management vendor, trading partner, or technical support area. They will assist you in enabling this feature for your practice or facility. This enhancement applies to both professional and institutional claim submissions.

What Are Acceptable 275 CA Transactions?

Electronic claim attachments (275 CA) are supplemental documents that provide additional patient medical information to the payer in support of your claim submission. The 275 CA enhancement — also known as ANSI ASC X12 275, Version 210 — will allow the batch submission of additional claim documentation. 

Common attachments include laboratory results, information from the member’s electronic medical record (EMR), list of medications, and certificates of medical necessity (CMNs).  

When to Submit a 275 CA Transaction?

Some claims require additional documentation to support accurate and efficient processing by Highmark. 

Why Submit a 275 CA Transaction?

Sending necessary attachments electronically offers a faster, more streamlined, and cost-efficient process over mailing or faxing an attachment. 

What Highmark Products are Eligible to Receive 275 CA transactions?

All Highmark products — including Commercial, Medicare Advantage and ACA — are eligible to receive 275 CA transactions. The inclusion of the correct Highmark NAIC code is critical for successful 275 CA transactions. NAIC codes can be found in Chapter 6, Unit 2 of the Highmark Provider Manual.
