House Call Program

The House Call Program is a supplementary service where CNPs (NPs for NY) go into a member’s home (or via telehealth) to administer an “in-home health assessment” and address open care gaps in support of Risk Adjustment and Stars initiatives. This service is coordinated with vendor partners Matrix Medical Network, Signify Health, and Optum.

Highmark encourages member/provider follow-up to review the outcome of the member’s health assessment, as well as any lab results.

  • The member’s in-home health assessment and lab results are shared with the member and member’s PCP.
  • Highmark continues to promote Annual Wellness Visits (AWVs) and other healthcare engagements with a PCP early and often.

These in-home health assessments are provided to Highmark Medicare Advantage members residing in AR, AZ, CO, DE, ID, MA, ME, MN, MO, MS, MT, NE, NH, NJ, NM, NV, NY (Northeastern), OH, PA, RI, SC, UT, VT, WA, WV, and WY.

Care at Home

Care at Home is a program offered to certain Highmark Medicare Advantage members in the Western and Northeastern New York markets. The program delivers compassionate care to eligible members with complex needs and multiple chronic conditions with the goal of ensuring better quality of life and positive outcomes. Care at Home is a voluntary program for eligible members.

As of January 1, 2025, ConcertoCare will manage the program, which is designed to collaborate and communicate openly with members’  primary care providers. ConcertoCare is committed to ensuring that individualized care plans are being followed at home between office visits. Care at Home does NOT replace primary care.
