Supporting Medicaid Members with Maternal Home Visitation Programs

Highmark Wholecare is collaborating with local community organizations across Pennsylvania to offer Maternal, Infant, and Early Childhood Home Visitation Programs (MHV). These programs are designed to support the well-being of families and young children, and providers are encouraged to refer Medicaid Members who would benefit.

These programs are designed to support families by:

• Improving the health of both mother and child
• Preparing children for school
• Promoting financial stability for families
• Preventing child abuse and neglect
• Addressing substance use issues

The key features of these programs include:

• Personalized Support: Each family receives a customized plan.
• Home Visits: Trained community health workers or other professionals visit families at home.
• Focus on Prevention: The programs encourage preventive care and healthy habits.
• Addressing Barriers: They help families overcome obstacles to access health care and social services.

We currently work with the following five vendors to provide these valuable services:

• Center for Population Health: Serving members in Cambria, Somerset, Fayette, Blair, and Bedford counties.
• Hamilton Health: Serving members in Dauphin, Perry, and Cumberland counties.
• Healthy Start Pittsburgh: Serving members in Allegheny County (specifically zip codes 15136, 15221, 15212, 15210, 15132, 15147, 15206, 15208, 15219, 15235, 15104) and Westmoreland County.
• Lifesteps: Serving members in Allegheny, Armstrong, Beaver, Butler, Indiana, Lawrence, Washington, and Greene counties.
• Tri-County Community Action: Serving members in Dauphin, Perry, and Cumberland counties.

How to Refer Members:

Please contact our Enhanced Member Supports Unit (EMSU) at 1-800-392-1147 to refer Highmark Wholecare Medicaid members. Our Case Management Team will:

• Connect with the member to explain the program
• Determine the most suitable MHV program for their needs
• Complete the referral process
• Coordinate services between the member and the MHV program for up to 18 months after a baby is born

Participation is voluntary, and members can choose whether or not to participate. DHS requires that these programs are available to all first-time Medicaid parents, parents/caregivers of children with identified risk factors (social, clinical, racial, economic, or environmental), and any infant/caregiver who requests support. Services are available from pregnancy through the child's first 18 months.
