Reportable Conditions

As a friendly reminder, Highmark Wholecare providers must report certain diseases, infections, or conditions as determined by 28 Pa. Code §27.21 et seq. of the Pennsylvania Code, Subchapter B. REPORTING OF DISEASES, INFECTIONS AND CONDITIONS. Highmark Wholecare has established The Reportable Conditions Policy to explain this contractual requirement as well as the methods by which providers will be notified of its necessity.
