Provider Self-Audit Form: Address Change

As a friendly reminder, federal and state regulations require providers to routinely audit claims for overpayments. Providers must notify us in writing and return overpayments within 60 calendar days. Provider Self-Audits (PSA)s may be submitted through the provider portal or mailed to us.

Starting July 1, 2024, the address to send PSA overpayment checks will change to:
