Our Special Needs Unit Has a New Name

Highmark Wholecare’s Medicaid Special Needs Unit (SNU) is now called the Enhanced Member Supports Unit (EMSU).

Even though we changed our name, our case management services remain the same! Need a refresher on what our case management teams at the EMSU can do to assist your patients? Here are a few examples: 

  • Education on disease management
  • Post hospital discharge planning
  • Medication reconciliations
  • Care Gap closures
  • Establish PCP and specialist care
  • Benefit education and coordination
  • Health related social seeds

Our trained nurses and social workers are ready to help your patients maintain their whole health. We will work with members to coordinate their physical, mental, and social needs. To learn more or to make a referral, providers can call our EMSU hotline at 1-800-642-3550.
