Opt-in the HWPE Incentive Program Today

As we highlighted last month, the 2025 Highmark Wholecare Practitioner Excellence (HWPE) Incentive Program launched on March 1. This program supports our mission to improve the health and wellness of the individuals and communities we serve by providing access to integrated, superior health care.

Providers must acknowledge that they are opting-in to the core HWPE program, the Maternity Quality Program, or both. Please contact your Clinical Transformation Consultant directly or email us at ProviderEngagementTeam@HighmarkWholecare.com for information on the opt-in process. Providers will be enrolled in their chosen programs based on provider specialty, eligibility criteria outlined in the HWPE Incentive Provider Guide, and network participation. 2025 HWPE Overview Webinar Series

The HWPE Provider Engagement Team is offering multiple, hour-long webinars to provide an overview of the 2025 HWPE Program from now through September. You can register today for one or multiple sessions by following the steps below.

1. Use the links below for the session(s) you would like to attend and click “Register”:

Wednesday, April 9th at 9:00 a.m.

Tuesday, April 22nd at 1:00 p.m.

2. On the registration form, enter your information and then click "Register.” Once the host approves your request, you will receive a confirmation email message with instructions on how to join the event.

2025 Care Gap Management Application and Provider Reporting

You are also invited to leverage our 2025 Care Gap Management Application (CGMA), which will launch on April 1. This application offers providers access to important care gap information. 

The CGMA has been designed to help providers by simplifying the flow of member care gap information between you and us. With this powerful, easy-to-use web application, you will be able to:

• View member care gaps.
• Submit evidence for care gap closure.
• View your progress towards closing member care gaps.
• View your Highmark Wholecare member roster.
• View your HWPE Performance and Opportunity Reporting.
• Bulk upload Evidence Submissions for a member.
• Bulk generate member level Gap PDF Reports.
• Generate multi-provider Gap Reports.
• 2025 Enhancement: Monitor due dates for applicable measures, for example W30 and OMW. Additionally, a badge has been added to measures with a due date based on an event. Remaining days to take action are displayed on the measure banner.
