Obtaining Consent to Treatment Forms
According to Medicaid and Medicare regulations and as reflected in Highmark Wholecare’s Medicaid and Medicare Provider Manuals, providers must obtain Consent to Treatment forms from patients. As a best practice, these should be obtained on an annual basis. The form documents a patient’s permission to receive treatment and ensures they understand any risks or alternatives available to them. The Consent to Treatment form is required by Pa. Code §1101.75 a (10) and 42 CFR.
