Important Credentialing Update

Attention: Audiologists, Speech Language Pathologists, Physical Therapists, and Occupational Therapists

Highmark Wholecare will now individually credential Audiologists, Speech Language Pathologists, Physical Therapists, and Occupational Therapists. As a result of this change, we are requesting that all Audiology, Speech Language Pathology, Physical Therapy, and Occupational Therapy entities complete the Credentialing Data Provider Form as soon as possible and fax the completed form to 1-855-322-2026.

Credentialing 10 or More Providers

If your practice has 10 or more providers needing to be credentialed, please complete the alternative Provider Roster as soon as possible and send the completed roster to credentialing_non-delegate_rosters@highmarkwholecare.com.
