Highmark Wholecare’s Disease Management Programs

Highmark Wholecare’s Disease Management Programs use a multidisciplinary approach that identifies health conditions within our population. The programs focus on identifying the needs of the population within these chronic conditions and perform scheduled outreach activities to provide education, resources, and care coordination to members.

The program focuses on providing disease management for the following conditions:

• Congestive Heart Failure (CHF)
• Chronic Obstructive Pulmonary Disorder (COPD)
• Chronic Kidney Disease (CKD)
• Inflammatory Bowel Disease (IBD)
• Diabetes

Condition management for our members supports the provider-member relationship and plan of care to prevent the exacerbations and complications that require emergency room and inpatient health care services.
