HWPE Incentive Program and Care Gap Management Application

The 2025 Highmark Wholecare Practitioner Excellence (HWPE) Incentive Program launched on March 1.  Our providers play a crucial role in promoting the overall health of our members, and we would like to welcome you to the HWPE Program to highlight the excellent care you provide. Last month, we outlined how providers can learn more and opt-in to the 2025 Program.

The deadline for HWPE program opt-in is September 30, 2025. 

2025 HWPE Overview Webinar Series

The HWPE Provider Engagement Team is offering multiple, hour-long webinars to provide an overview of the 2025 HWPE Program from now through September. You can register today for one or multiple sessions by following the steps below.

1.) Use the links below for the session(s) you would like to attend and click “Register”:

• Friday, May 9 at 9:00 a.m.

• Tuesday, May 20 at 1:00 p.m.

• Wednesday, June 18 at 9:00 a.m.

• Tuesday, September 23 at 1:00 p.m.

2.) On the registration form, enter your information and then click "Register.” Once the host approves your request, you will receive a confirmation email message with instructions on how to join the event.

2025 Highmark Wholecare Care Gap Management Application & Provider Reporting Launch

Additionally, last month we announced the launch of the 2025 Highmark Wholecare Care Gap Management Application (CGMA). This application is live as of April 1, 2025, and helps simplify the flow of member care gap information between you and us. With this powerful, yet easy-to-use web application, you will be able to efficiently view care gap information, submit evidence of care gap closure, review rosters and reporting, and more. Additionally, the new 2025 application now allows providers to easily monitor due dates for applicable measures, such as W30 and OMW. The deadline to submit evidence for open gaps in the CGMA is Friday, January 9, 2026.
