Diabetes Support with Good Measures

High-touch, high-tech approach for Diabetes support

The Diabetes Support Program through Good Measures offers self-management education and support to members with Type 1 or Type 2 Diabetes as part of their Highmark Wholecare plan. Education is provided through 9 segments, including information around nutrition, exercise, medications, monitoring, and complications. Members also have access to an app to make optimal food choices, message their personal Diabetes coach, and track health goals.

Recognized program through the American Diabetes Association

To be eligible for this program, patients must be a Highmark Wholecare member, ages 18 and older, and have a Type 1 or Type 2 Diabetes diagnosis.

It’s easy to refer patients! Providers can refer members simply by submitting the Good Measures online form.

Good Measures is a separate company that administers the Diabetes Support Program for Highmark Wholecare.
